Mindy L. Chmielarz

Mindy L. Chmielarz is a disability insurance lawyer and founding partner of Disability Insurance Law Group, representing individuals in private and group insurance benefit claims throughout the country. Her practice focuses on disability insurance, long-term care insurance, life insurance, and ERISA-governed benefit disputes. She handles initial claims, administrative appeals, denied benefits, lump-sum buyouts, litigation, and related insurance matters. Her work includes state and federal court proceedings involving disputes with major insurance carriers.

Mindy earned her J.D. from the University of Miami School of Law, graduating magna cum laude. She served as an assistant editor of the Law Review and as a Dean’s Fellow. She earned her M.A. in Political Science from the University of Florida. Her graduate honors included a teaching award and service as a graduate assistant to Lawton Chiles. She earned her B.A. from the University of Michigan, graduating with honors.

Mindy focuses exclusively on representing insured individuals seeking benefits under disability, long-term care, and life insurance policies. She counsels clients applying for disability benefits and represents claimants appealing denials under individual and ERISA plans.
